Included Services

  • Detailed Annual Physical Exam

  • Routine Primary Care Office Visits

  • Visits for acute injuries or illnesses, including X-rays, breathing treatments, splinting, and suturing

  • Preventive Medicine and Counseling

  • Creation of a comprehensive lifestyle plan for optimal health and wellness

  • Diet and Exercise Counseling, including BMI and Body Fat determination

  • Routine lab work

  • EKGs and chest X-rays

  • Discounts for COVID-19 testing

The above services are included in your membership. Note that for office visits there will be no charge! There will be no time wasted on verification of benefits, or discussion of co-pays, co-insurance, or deductibles. Some medical equipment, such as splints and slings or orthopedic boots, will be charged for at the time of the visit, but we will strive to keep your cost as low as possible.

(ages 26-64):

$130.00/month

(ages 18 -25):

$80.00/month

or $960.00/year

We can not accept Medicare patients at this time, but we are working to do so in the future.  Please check back with us.

Memberships can be paid annually in a single payment, or monthly via automatic credit card draft.  You may cancel your membership at any time with 30 days written notice. 

You may be able to use your HSA funds for membership payment.  Please check with your tax accountant  and/or HSA administrator to confirm.
